In this key role as a Community staff nurse within our dedicated 5-19 years team, you will be providing exceptional care to our children and young people. You will be responsible for operational and strategic management of a defined caseload, ensuring that high-quality care is delivered in line with the Healthy Child Programme.

The successful candidate will need to be role models and demonstrate a willingness to lead, support and implement change, while adhering to our trust values, 'we listen, we care, we deliver'. We pride ourselves on delivering high quality, patient centred care and encourage learning and training to get the best out of our employees., The post holder will work as part of a 0-19 Children's Services team and will beintegrated and co-located with the Local Authorities Prevention and Early Help team,delivering services within an integrated care pathway.

Under the supervision of the Health Visitor/School Nurse the post holder will supportthe delivery of the Healthy Child Programme within a progressive universal model ofservice delivery dependant on assessed needs.

Using best available evidence, the post holder will work in partnership with keypartners and communities to deliver targeted interventions to improve the health andwellbeing of children, young people and families to address health inequalities,especially in those from disadvantaged groups
