Full Job Description

We are looking to recruit new Young Person Patient Partner Volunteer for our CYP Board, to ensure we have young person representation at our CYP Board, as we value and need the input of young people that use or have used our services within the hospital and community setting. This is a voluntary role, and our Patient Partners have expenses reimbursed.
The aim of the CYP Board is to ensure hospital and community services meet the needs of children, young people, and their parents / carers, and provide effective and safe care, through appropriately trained and skilled staff working in suitable, CYP friendly, and safe environments.
The CYP Board is made up of people with a shared interest in ensuring that the needs of CYP are met within the hospital and community health care setting and central to work involving the design, delivery, and improvement of services. The members of the board include:
+ Doctors, nurses and allied health professionals from the hospital and community setting as well as representatives of team such as patient experience, Child and Adolescent Mental Health Services (CAMH's) and Achieving for Children (AFC) services.
+ Nursing leads from the hospital and community, who see and treat CYP
+ Parent Patient Partner.
The meeting is chaired by Sarah Shade, Deputy Chief Nurse for Kingston Hospital.
How often are the board meetings?
The CYP board meets 4 times a year and the meeting are scheduled for an hour and half (11:00 -12:30) and are currently virtual meetings on MS Teams. The dates for 2024 are as follows: -

+ 13 November 2024
Essential Information:

The Mandatory training for this role will be on Wednesday 24th July 2024.

+ Driving Licence photo card (UK, IOM or CI)
+ Current EU Photo Driving Licence
+ EU national identity card - EU only
Group Two:
+ Driving Licence photo card (UK, IOM or CI)
+ Current EU Photo Driving Licence
+ Driving licence - old-style paper licence (must be UK)
+ Bank statement (UK/EEA) - less than 3 months old
+ Utility bill - less than 3 months old (UK)
+ Utility bill - 2nd Supplier - less than 3 months old
+ Council Tax Statement - issued in the last 12 months
+ Mortgage statement (UK or EEA) must be a recognised lender and issued in the last 12 months.
NB: If you are aged 16 - 19 years, in full time education and you are unable to provide two documents from group 2 above, the following ID options can be used:
+ Full UK birth certificate (A4 size original , not photocopy, containing mother and fathers details)
