Urgent Care Response Physiotherapist | Norfolk Community Health and Care NHS Trust

This is a really exciting opportunity for a Senior Physiotherapist, or aspiring junior physiotherapist looking to step into a senior role, to be part of our North Norfolk Unplanned Community Response team. We provide admission avoidance and urgent community response to individuals in crisis in their own homes.

You will be part of a team which provides , effective short term reablement/rehabilitation programmes to patients in their own home following recent hospital discharge, clinical triage as part of the NEAT (Network Escalation Avoidance Team) referral pathway and respond clinically to admission avoidance and support patients recently discharged home, with referrals coming from community services, the ambulance service and front door services at the acute hospitals. Be part of the Virtual Ward development in North Norfolk and work alongside the MDT to support discharges from hospital.

This is a 7 day a week service, weekend and Bank Holiday working will be expected. Hours of service delivery, 8am-8pm. To demonstrate a flexible approach, being able to adapt and respond effectively and efficiently in potentially stressful case scenarios. The post holder will use their Physiotherapy diagnostic skills within their assessment to determine need for further investigations, and to support the urgent Community Response Team with ongoing patient management in order to prevent avoidable admission to hospital and facilitating early supported discharge.

To lead in the delivery of high standards of Physiotherapy interventions to patients within their own place of residence to prevent avoidable admissions to acute care. They will provide short term reablement/ rehabilitation programmes with effective short term goal setting for vulnerable adults with acute health needs. This will involve using health coaching skills to effectively engage with the patient and their family to ensure expectations are appropriately managed. To lead in the delivery of high standards of health care to patients within their own place of residence by assessing, planning, implementing and evaluating care in order to prevent avoidable admission to hospital and facilitating early supported discharge.

This role will predominantly involve face to face intervention, with an element of Triage for the Urgent Community Response Team on a rota basis.

The post holder will show a level of resilience by working with the team to develop competencies in all areas of practice. Each clinician within the service has developed competencies in Physiotherapy, Occupational Therapy and Nursing. It would be an expectation for the post holder to be involved in the training of other team members to maintain competence within some aspects of Physiotherapy, as well as develop and maintain own competencies in areas of Occupational Therapy and Nursing.
