Full Job Description

Exciting opportunity to join our established Home 1st Twilight Nursing team in Milton Keynes. We are recruiting staff to work alongside our qualified nursing team, delivering out of hours, unplanned care to patients in Milton Keynes.

Twilight operational hours provide flexibility for care responsibilities outside of work.

In this role you will ensure the most appropriate care is delivered to patients in their own homes, whilst working alongside registered nurses.

Full driving licence and access to a car for work is essential to this role.

Home1st is based in Bletchley and the modern office is a central hub for multi-disciplinary working. You will be provided with uniform and have mileage expenses paid for., To work as a Health Care Assistant to support the multidisciplinary team, in the effective and efficient provision of care to aid patient's recovery, promote independence, health and wellbeing following sudden illness or deterioration.

Provide support to registered nurses, working as a team providing individualised care plans as set by nurses.

Work as a member of the multidisciplinary team observing, recording and reporting any changes in a patient's condition or circumstances to registered staff.

Be familiar with the steps necessary in an emergency and be able to act accordingly.

To provide planned, delegated nursing interventions for patients in their own homes or other settings as appropriate.

To participate in shared care with other agencies / services as required.

Provide accurate feedback on patient's care to Nursing teams.

Ensure the needs of carers and relatives are listened to and reported back to the Nurse in Charge of the shift.

Ensure clear, concise and accurate records are kept and updated in a timely manner, including the use of departmental electronic records.

To be aware of health and safety and follow risk assessments to ensure the safety of service users and staff.

To undertake relevant training as provided.
