Full Job Description

Join our dynamic and progressive healthcare team as a trainee Advanced Nurse/Clinical Practitioner and take your career forward as a developing practitioner. As an integral member of our clinical team, you will play a crucial role in evolving our NGHT Frailty service and contribute to our exceptional care in the Emergency Department., As a trainee AN/CP you will have the opportunity to develop under an experienced medical team identifying Frail patients, undertaking patients' clinical history, performing clinical assessments (understanding the physiology impact of ageing) develop plans of care and co-ordinate safe effective discharges. Working alongside allied services., The Frailty team work in partnership with the Emergency Department triaging patients' ready for assessment and referral to the Frailty Unit or preparation for discharge.

The team undertake full holistic assessments providing an all inclusive comprehensive Frailty service. The trainee will be fully supported by the Frailty Team when undertaking advanced levels of clinical practice, knowledge and skills.
