Full Job Description

We are recruiting a Clinical Support Worker/Therapy Assistant to support the Occupational Therapist on therapy-based assessment and treatment as part of our Discharge to Assess team based at the Royal Lancaster Infirmary.

The team will work to deliver a Discharge to Assess service. The service is supporting residents within the Morecambe Bay and South Lakes locality. The service provides assessment for patients on discharge to provide assessment within the person's home rather than in hospital, as part of a Discharge to Assess service.

You will join a dedicated and motivated team of Occupational Therapists, Physiotherapists, Rehabilitation technical instructors to support and identify discharge requirements, making referrals on to Health and Social care community services.

All assessment will be within the patient's home at point of discharge from hospital.

You will have previous experience within a health and social care setting eg: acute inpatient, admission avoidance or community-based rehabilitation services.

The ability to travel to a variety of venues throughout the community to deliver patient assessments within an identified locality.

In addition to a comprehensive induction package we offer Team support, staff supervision and in-service training opportunities

We operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community health care premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.

FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.

WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.

All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
