Full Job Description

The community rehabilitation team supports people to reach an optimum level of function; focussing on activities of daily living, vocation and leisure and the post holder will work towards these goals and:
+ Provide care for people both in the community and for those planning discharge from the Rehabilitation Inpatient units with severe mental illness who have multiple and complex needs under the direction of qualified practitioners.
+ Provide support to an allocated group of service users, promoting their recovery and social inclusion and maintaining them in their community environment (STR role).
+ Focus on the direct needs of service users, working across boundaries of care, organisation and role, coordinated through the Integrated Care Programme Approach (ICPA).
+ Support the service user in assessment and planning and implementing their care plan
This will be a flagship service enabling Dorset HealthCare to become one of the best rehabilitation services across the country.
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

Do you want to be part of a service and help shape the future of the Mental Health Rehabilitation Services?
We currently looking for a Support Time Recovery Worker Band 3 for our Community Rehabilitation Team (CRT) based in the West of the county.
Our vision is to provide a community team in the East and one in the West of the county. The CRT West is currently working out of the Forston Unit in Dorchester, however due to the size of the catchment area, we work a hub & spoke model and provide hot desking across the county with a main hub. There is also an element of homeworking factored into this team and as such all staff will be provided with appropriate IT equipment.
All post holders will work as part of an integrated multi-disciplinary team that includes Mental Health Practitioners, Occupational Therapists, Psychologists, Psychology Assistants, Support Time Recovery Workers, Employment Specialists, Social Workers, Care Managers, Admin Assistants, Advanced Nurse Practitioners and Medical Staffing.
The CRT supports people who have chronic and disabling mental health problems that may be treatment resistant and who have previously spent long periods in hospital.
We are looking for people who are reliable, committed and dedicated, who have excellent communication and organisational skills and the ability to prioritise their work.
