Full Job Description

Cedar's ward is a 10 bedded unit for male older people with organic illnesses. We aim to offer the best care we can provide to both patients and those that support them. We will work with families and carers on a discharge pathway back into the community, into their own homes and where this is not possible, assist in a smooth and dignified transfer into permanent care.

The post holder will be expected to undertake all aspects of the role identified in the job description and meet the requirements of the person specification. The role will involve delivering person centred care to the patients on the ward. You will be responsible for a wide range of tasks such as care planning and implementation, risk assessment, physical health assessment, liaising with the wider MDT, participating in ward rounds and fulfilling the role of named nurse.
