Full Job Description

The post holder will work as a member of the Multi-disciplinary Team ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting care and treatment to young people. Planning & Interventions, Reviews and Discharge Planning. You will act as a named nurse and support band 3 nurses within their role and within multidisciplinary team, developing coproduced, patient centred needs based care plans, through assessment, planning and implementation and evaluation.
You will provide excellent professional, skilled and effective person centred evidence based nursing care. You will be actively involved in the health and wellbeing of staff, service users, families and carers. You will be visible and accessible to both patients, families and carers, working together to ensure efficient, effective safe and caring services. You will contribute to all performances indicators, essential standards and quality outcomes. You will contribute to the education and development of the workforce and students. You will participate in staff appraisals, supervisions and support personal development plans. You will safely administer medications following trust polices and practice guidelines. You will provide compassionate care that is based on empathy, kindness, respect and dignity. As a band 5 you will also be involved in link roles and supporting clinical nurse leads
