Specialist Physiotherapist (Community)

More Detail about the role: The Community Therapy Team work with people with a wide range of disabilities and acute or long term conditions striving to deliver excellent patient centred care. Our work is consistently varied and stimulating covering admission prevention and supporting discharge, and rehabilitation in a range of community settings through medical, social and public health approaches. The Community Therapy Team works within the broader Health and Social Care team alongside colleagues from Community Nursing and Adult Social Care. We are a positive and proactive team and adapt to the changing needs of the patients we work with and the service demands. If you are flexible and creative in your approach to work, enjoy working in a multi-disciplinary team, have an interest in developing yourself and others, this could the opportunity for you. Working Pattern: 37.5 hours per week.
