Full Job Description

This role is office based, Monday - Friday 9am to 5pm, however there is an on call element that will be required, which is undertaken from home. You will be provided with a work laptop and a work mobile phone in order to carry these duties out.

The aim of the position is to enable customers to remain safely in their own homes with as much independence as possible. Alongside ensuring, through regular communication, review and continuous monitoring, that care and support are provided at all times in a professional, discreet and caring manner, always ensure all customers maintain the right to privacy and dignity. This includes maintaining excellent communication with key professionals and providing a prompt response in a professional manner.

To ensure care services are delivered in accordance with current best practices, according to policy and procedures, agreed standards, legislative requirements, and relevant regulations under the direction of the Specialist Care Registered Manager, and within the financial plans agreed from time to time.

To continue to build on the skill sets of the staff and adhere to CQC standards relating to compliance, and undertake such training and development as may from time-to-time be required to maintain practices as up-to-date.

To organise and supervise the Specialist Care Support Workers within SCA Care in accordance with agreed standards.

  • Develop effective working relationships with the staff who are in remote roles.

  • Work to ensure staff have continuity in their rotas and 90% of the rotas for customers are in a rolling template.

  • Ensure staff compliance in training and HR file is above 90%

  • Audit the electronic systems on a weekly basis and carry out any required actions to rectify action plans

  • Complete formal supervision of the Specialist Care Field Care Supervisors you are accountable for, in a professional manner at monthly intervals.

  • Assist in the development of the philosophy, goals and objectives for the care practice.

  • Assist in the assessment of the effectiveness of care implementation and delivery

  • Implement actions to meet and maintain care standards.

  • Work in cooperation with members of multi-disciplinary health teams so as to maximise opportunities for customer care.

  • Ensure that the care service is delivered in a safe and person-centred manner.

  • Ensure customer rights are protected and encourage a model of self-care and customer rehabilitation.

  • Evaluate care plans where necessary.

  • Carry out the 'on-call' on a rota basis to ensure that emergency support is provided as and when required.

  • To work at a pace that ensures you're able to adhere to the commissioner's requirements whilst complying with CQC regulations.

  • Notify the Specialist Care Registered Manager of any changes to the rota requirements of customers.

Although a Level 3 QCF/NVQ is essential, we are also interested in those who can uphold our values and can demonstrate these to not only our staff but also our customers in all encounters. Ideally with ability to work well under pressure, and with an engaging and energetic nature, this role would suit either an existing Coordinator looking for a new challenge or an experienced Senior Care Worker looking to take a step up.
