Are you enthusiastic, resilient and above all passionate about enabling people to live their 'best life'?

Do you want to work in a truly person-centred way that embodies strength-based approaches so people who need support can live or access short breaks in the community within a family/home setting?
We're looking for a Social Worker to join our well-established Shared Lives Team.
Shared Lives Social Workers are a key part of the team and if successful you will be:
+ Working with your own caseload of Shared Lives carers to ensure they meet the requirements of the scheme, relevant legislation and CQC guidelines.
+ Assessing applicants that wish to become Shared Lives carers
+ Providing support to Shared Lives carers, carrying out home visits, finding solutions to challenges that may arise and undertaking annual Shared Lives carer reviews.
+ Participating fully in the matching process.
+ Producing high quality personal service plans, risk assessments and risk enablement plans.
+ Promoting Shared Lives through internal and external networking and promotional events.
You must be an organised person who can manage their time well, an excellent communicator who can use their inter-personal skills to develop and maintain effective relationship with our Shared Lives carers and other members of the Team.
The Team is small but very supportive, with a unique position across both social work and direct care services. In joining the Team, you would receive a full induction, regular supervision, opportunities to develop your practice and see first-hand the difference Shared Lives can make.
