Social Worker

We are urgently seeking for dedicated social workers to join our team. As a Social Worker, you will play a crucial role in providing support and assistance to individuals and families in need.,

• Conduct assessments to determine clients' needs and develop individualized care plans for adults.


• Provide counseling and emotional support to clients facing various challenge


• Connect clients with appropriate resources and services within the community


• Advocate for clients' rights and best interests


• Monitor and evaluate clients' progress and adjust care plans as needed


• Collaborate with other professionals, such as psychologists, healthcare providers, and social service agencies
  
  Must have at least 1+ years of work placement/work experience within social work.


• Must be registered with Social Work England and will need to be proved at the application stage.


• Must have a drivers license for on-call shifts or live fairly close to DE11 0AQ.
