Senior Mental Health Nurse Practitioner

The post holder will be a Registered Mental Health Practitioner, with at least 2 years community experience of managing a complex caseload of patients. You will be responsible for a caseload of patients with complex health and social care needs. You will work autonomously to case manage these clients and use advanced nursing assessment skills, to manage their conditions in the community. He/She will work as part of the health inequalities clinical team, working within and across professional and organisational boundaries. Liaising with GPs, other professionals and statutory and voluntary agencies he/she will deliver individualised care to patients.
