Full Job Description

The immediate priorities for the postholder will be to:

  • Take clinical ownership for the development of a South East London approach to community based care that realises the Fuller stocktake vision, reduces health inequalities and improves care for patients with complex needs. This needs to be built bottom up, working with and across our six Local Care Partnerships.

  • Building relationships and championing collaborative approaches with trust partners and enabler teams that lever in the support required to enable Integrated Neighbourhood Team working in all places across South East London

  • Providing senior clinical leadership over the development of workforce strategies, plans and programmes which increase the sustainability and resilience of the primary and community workforce and which promotes a culture of integration.

  • Taking overall clinical accountability for the agreed priorities of the CBC directorate, in line with the SEL Joint Forward Plan working closely with the Director of Community-Based Care, this will include representing the ICB at regional and national networks as required.

    Clinical and Care Professional Leadership (CCPL) roles have been established in SEL ICB since April 2022 and have been a valuable source of clinical leadership.

  • Within the Community-based Care (CBC) directorate, the Clinical and Care Professional Strategic Lead for Community-based Care has a significant role, providing system clinical leadership over the development of integrated community based models across South East London that deliver and enable population-driven, proactive and personalised care.
