Full Job Description
- 4-6 session per week
- SystmOne practice, using Ardens and Accurx
- Urgent care team offering support on duty days.
- 12 appointments per routine clinical session
- Team of nurses/HCAs providing nurse-led chronic disease management.
- Practice paramedics providing a visiting service
- Award winning PCN Pharmacist Team
- PCN first contact physiotherapy, social prescribing link workers & mental health practitioner
