It is an exciting time at The Norfolk Hospice Tapping House as we are delighted to be relaunching our invaluable Hospice at Home Service. The Hospice promotes care and support that are delivered in a culture that respects patient autonomy and is centered around individual end of life care wishes. Caring for patients who are in the last weeks of life is a specialist and vitally important service. When delivered well, with compassion, knowledge and shaped by what is important to the person it is a powerful and moving experience. We only get one chance to get it right for the patient and their family and our team go to extraordinary lengths to ensure we get it right every time. We are looking for a Registered Nurse (Band 6) to join our dedicated team. Is this you?, The post holder will be responsible for delivering the highest standard of palliative care to patients and their families under the care of The Norfolk Hospice Tapping House. Care is provided in the community across 24 hours a day, seven days a week and aims to support patient choice and access to excellent end of life care and support. As a core member of the care team and a member of the wider multidisciplinary team, the successful candidate will be involved in assessing and developing appropriate plans of intervention and support for patients and their families, providing advice and signposting to other services as required as well as ensuring effective, timely communication. Whilst this role is part of our Hospice at Home team you may be asked on occasions to work on our Inpatient Unit. As a Band 6, there is an expectation of experience of leadership of shifts, with the support of the ANPs, Ward Manager and Hospice Doctors to ensure effective flow of patients
across the services, as well as providing mentoring and role modelling for less experienced colleagues. You will be involved in helping to facilitate the education and support of qualified and unqualified staff and to participate in the development of nursing practice across the hospice. Care provision shall reflect evidence based practice, in line with legal requirements, statutory rules and Hospice policies relating to practice, and the NMC Code of Conduct.
