Full Job Description

We are looking for full time experienced Recovery Coordinator to join our team of Recovery Coordinators including GP Shared Care and Aftercare at Primary Care Recovery Service (PCRS) in Lewisham.

You will manage a caseload of 50+ adult services users experiencing range of substance misuse issues especially alcohol and non-opiate, devising, implementing, and reviewing ongoing recovery and treatment plans, liaising closely with the full range of workers and skills within the service team and other agencies within the borough of Lewisham. problems in the borough of Lewisham.
You will be working with Non-opiate and Alcohol clients completing triage assessments, one-to-one psychosocial key working sessions, Care Plans, Risk Assessments, TOPs and overall supporting services users in their journey to recovery.
You will help our services users set SMART goals and then achieve their goals in a safe and supportive way. Recovery Co-ordination is a diverse and rewarding role.
You will be expected to liaise with other treatment providers including health care professionals, adult and children social services within the borough and attend professional meetings including weekly MDT team meetings.
Key Duties:
Manage an adult caseload of non-opiate and Alcohol clients
Devising, implementing, and reviewing ongoing recovery and treatment plans.
Facilitate and promote individuals' progression from initial engagement, through structured treatment, to self-sustained recovery.
Reduce alcohol-related harm to individuals and the wider community.
Working collaboratively with key partners and colleagues to achieve the best health and well-being outcomes for individuals.
