An exciting opportunity has arisen for a Navigator to join our South Yorkshire RECONNECT service.

This new post requires someone with drive, energy, and a commitment to deliver high performance and standards. The postholder will be joining a supportive service line management team, with supervision and opportunities to participate in service development.

The postholder will have excellent communication and person skills and have experience of working with individuals in mental health, learning disability or the criminal justice service.

RECONNECT is a care after custody service commissioned by NHS England to offer support and release planning to individuals aged 18 and over with an identified health need, to reduce inequalities and address health-related drivers of offending behaviours.As a Navigator, you will support the transition of people into community-based services, thereby safeguarding health gains made whilst in prison or immigration removal centres (IRC) for up to 6 months post-release date, or when all health care needs are met, whichever is the soonest.

RECONNECT offers liaison, advocacy, signposting, and support to facilitate engagement with community-based health and support services.

The South Yorkshire RECONNECT Service will cover HMP Doncaster, HMP Hatfield, HMP Lindholme and HMP Moorland. The successful candidate will be primarily based within the community setting., The successful candidate will be part of a new dynamic team. We are evolving as a service to provide positive health outcomes for people within the criminal justice system. We work with vulnerable people that have often experienced trauma in their lives. Some of the skills and qualities would include:

be responsible for your own caseload within their speciality area as an autonomous practitioner. In doing so, you will assess, plan, deliver and evaluate patient centred programmes of care to a specialist level ensuring that you practice within your sphere of competence and knowledge. The postholder will work to professional and regulatory body codes, standards and guidance at all times ensuring that their practice is grounded in evidence based theoretical and practical knowledge

approach each individual with care, compassion and sensitivity ensuring that these values are reflected in all aspects of their role

engage and actively involve the individual and, if appropriate, their family/carer, in their programme of treatment and/or care and its review by using person-centred techniques to promote a culture of self-care, involvement and empowerment

manage your own workload and that of others and understanding the boundaries of their practice. Be responsible and accountable for clinical administration tasks and management of resources within the clinical area e.g., stock control, specific clinical tasks to support the smooth running of the service
