Full Job Description

To undertake the duties of a practice nurse within primary care setting and to ensure that all nursing care delivered to the practice population is relevant, of a high standard and is effective and maintains responsibility for the nursing components of care.

Responsibilities

Clinical Nursing Practice

  • Provide wound care management using the appropriate referral guidelines.

  • Vaccinations and Immunisations and Travel Health Advice.

  • Monitor blood pressures

  • Care and Programme Management


  • Respiratory Care
  • Manages newly diagnosed patients with asthma and COPD

  • Maintains a disease register

  • Work within agreed local/national guidelines and patient group directions to manage patients with asthma, aiding diagnosis, reviewing inhaler technique and advising on treatment changes.

  • Family Planning and Woman's Health

  • Have a knowledge of family planning service and able to advise on methods of contraception, including LARC. To consult according to practice protocols with the support of GPs.


  • To provide a sexual health screening service to female asymptomatic patients according to national guidelines and local protocols.

  • Undertake cervical cytology and promoting breast awareness according to local and national guidelines.

  • Develop and maintain practice register for secondary prevention of CHD

  • Works with patients to set action plans for changes of lifestyle

  • Works within agreed protocols when managing risk factors to enable patients to manage their own disease.

  • Provides support by discussing interventions and treatments.

  • Maintain the consistency of professional standards set and take appropriate action if standards are not being met

  • Recognise the development needs of others and take appropriate action

  • Work within a team towards common objectives.

  • Support other members of the team.

  • Be aware of current evidence based approaches to patient care and demonstrate this in practice.

  • Demonstrate clinical expertise in practice.

  • Undertake administrative tasks related to the role

  • Participate in Audits
