Full Job Description

The PCN manager is a lead role within a complex environment of primary care management. They will deliver on a wide range of business, data submission, transformation, and improvement initiatives across the PCN providing strategic support to the board and its practices. the Board and its Practices. Working as PCN Manager in Winchester Rural North & East PCN you will be working with 6 member Practices; Alresford, Gratton, Stockbridge, Watercress, West Meon and Whitchurch Surgery supporting around 45,000 patients.

As a PCN manager you will develop strong relationships and engage with a wide range of stakeholders across the locality to ensure collaborative approaches to service development and delivery. In doing so they will improve integration with the broader community services as our network continues to mature.

You will be responsible for managing agreed projects with a focus upon developing the partnerships across primary care, community services, social care, mental health, the voluntary sector and secondary care to improve patients quality of care.

You will be responsible for disseminating new information across the member practices and tasked with coordinating shared learning events. They will actively support integrated neighbourhood ways of working whilst promoting standardisation for efficiency purposes. This role will be outward looking to help secure our resilience by actively seeking development opportunities and by coordinating proposals.

You will be essential to supporting the recruitment new ARRS roles into our primary care teams

The following are the core responsibilities of the PCN Manager:

  • To manage the development of Winchester Rural North & East PCN as a vehicle for the delivery of the Primary Care Networks objectives.

  • Be the first point of contact for Practice Managers across the 6 member practices regarding delivery of PCN DES, projects/schemes.

  • Be responsible for facilitating/coordinating/managing the delivery of PCN DES and projects.

  • Manage NHSE and other contractual requirements on behalf of Winchester Rural North & East PCN.

  • To develop and establish strong collaborative relationships with partners and stakeholders.

  • To support the strategic and policy development of Winchester Rural North & East PCN.

  • To develop a business plan for the successful delivery of the PCN DES obligations, and wider PCN (s) objectives.

  • Provide support to the Clinical Director to produce appropriate documentation for the PCN.

  • To support the Winchester Rural North & East PCN Leadership team in business case development such as bid-writing/submission as new opportunities emerge.

  • Develop innovative schemes, using best practices, to meet PCN objectives, exercise initiative and problem-solving within the remit of the project.

  • Be responsible for escalating issues at the board level following the PCN hierarchy structure for HR, Finance, Governance and Operational.

  • Make day-to-day judgements at a management level to ensure business and service continuity.
