Full Job Description

To accept or decline referrals based on CCT eligibility criteria, including a primary risk assessment. To arrange home assessments in liaison with the Case Coordinator. To complete holistic home assessments, and co-design Personalised Care and Support plans based on what matters to the person, family, and carers. Provide specialist assessments and interventions to prevent hospital admission and improve independence at home. To carry out intervention and treatment plans using evidence-based practice. To identify and utilise valid and reliable outcome measures, to assess baseline functional ability and follow-up assessments to identify and track individual progress. To independently manage a caseload and work as part of a team, meeting competing demands and priorities daily. To carry out ongoing risk assessments, complete safeguarding referrals and liaise with Adult Social Care to ensure patients needs are met. To signpost or refer, using knowledge and professional reasoning to justify and
advocate, to ensure the patients needs are met. Develop a range of therapeutic activities to benefit CCT service users. To record care plans for all patients, ensuring records are accurate and recorded promptly, on SystmOne and OneDrive. Provide specialist OT perspective to the MDT regarding assessment, treatment, and caseload management. To provide regular feedback to GP practices by recording and sharing outcomes, flagging any concerns, and working collaboratively to meet the patient needs, attending practice MDT meetings when required. To assist the Senior Psychosocial Practitioner in developing and maintaining good working relationships with GP practices, adult social care, hospitals, pharmacists, and voluntary and statutory agencies. Contribute to the support, training, and skill development of the CCT team. Assist the team lead in the day-to-day management of junior colleagues regarding clinical supervision and support.
