Full Job Description

We have an exciting opportunity for a Nursing Associate to join our Primary Care Network team. The Nursing Associate will work under the supervision of the Practice Nursing Team within the allocated PCN.

Leadership and support will be provided by the PCN Manager, Clinical Lead, and member practices. The Nursing Associate will work within the community to support the everyday running of the PCN practices.

Nursing Associates are equipped with the knowledge, understanding, skills, attitudes, and behaviours to support general practice. The Nursing Associate will assist with the delivery of health promotion, phlebotomy, chronic disease management, immunisation and vaccinations and other clinical roles in line with their training and level of experience.

The Nursing Associate will have a key role in supporting delivery of the new Network Contract DES Service specifications., Work within the PCN to provide and monitor care, alongside the wider PCN team, contributing to the delivery of excellent patient care, supporting the existing workforce under direct or indirect supervision

Improve safety and quality of care at every opportunity

Undertake additional clinical training as required by the PCN to support the Network Contract/DES Services Specification

Support registered nurses and the wider team to enable them to focus on the more complex clinical care

To support with chronic disease clinics and maintaining registers, deliver patient care, referring patients to other members of the PCN or to secondary/specialist care as appropriate. To implement and evaluate individual treatment plans dependant on training and experience

Provide support to the PCN with regards to their Network Contract DES Services Specification including CVD prevention and tackling health inequalities

Develop relationships across the PCN to support integration of the role across health and social care: including community care, secondary care, and mental health services.

Based in West Norfolk, our PCN serves over 55,000 patients across our seven member Practices of; Manor Farm Medical Practice, Plowright Surgery, Campingland Surgery, Litcham Health Centre, Howdale Surgery, the Hollies Surgery and Bridge Street Surgery.
