Full Job Description

To assess, plan and implement care to meet the identified individual health care needs of CYP, and families (this will include provision of continuing care support). To work in partnership with other care providers to facilitate, a prompt and efficient discharge from hospital, subsequently reducing hospital admissions and unnecessary re-admissions. Working in partnership with the family, Consultant Paediatricians, Nurse Consultants and key agencies, from both the statutory and voluntary sector. To participate in the risk analysis and decision making process at child protection meetings, providing written reports and giving health advice where needed. Initiate and develop flexible and innovative approaches to meet the needs of CYP and their families. Following a period of induction to participate in telephone out of hours advise service and 24/7 support when delivering an episode of active palliative care on call. To maintain a professional attitude at all times, developing the role
within the Community Childrens Nursing Team, (CCNT) in the best interests of the profession and in accordance with the NMC Code of professional conduct. Also providing on-going evidence of eligibility to practice. To assess and identify equipment which is suitable for CYP and family needs following the medical devices and procurement policy. To be flexible to work within a 24 hour shift pattern, including weekends and bank holidays. Knowledge, Skills and Experience Participate in Induction, training and preceptorship programmes for qualified nursing staff, including student nurses, healthcare support workers and as agreed with other agencies. To participate in relevant nursing forums to establish links. To be actively involved in the development of standards, protocols, care pathways and clinical audit. To be in involved in providing training to enable staff from multidisciplinary & multiagency partners to meet agreed competency levels. To provide clinical supervision and support to
staff, one to one and group supervision and carry out personal development reviews as required. Develop own and others knowledge and practice and organisational boundaries by providing clinical advice as required. Contribute to monitoring quality and the investigation of incidents. Update and maintain professional knowledge in relation to your area of interest around the national and local modernisation agenda for children, young people and their families. To be aware of own learning needs and limitations and to maintain professional expertise by embracing the concept of lifelong learning. To work alongside the clinical lead to support the operational running of the CCNT. (e.g. Sickness return to work interviews etc). Measurable Results Areas To undertake risk assessments, complete the necessary documentation and take appropriate action including informing your line manager. Utilise all resources efficiently and effectively within the locality. The post holder will
maintain accurate and contemporaneous records, deal with highly sensitive information respecting confidentiality and security at all times in accordance with Trust policies and data protection. To be involved with and/or support staff in auditing services and care practices on clearly identified areas as per Trust audit guidelines. To ensure that all clinical incidents and national patient safety alerts are reported, actioned and reviewed in a timely manner. To be responsible for and provide evidence that all mandatory training is undertaken & up to date as per Trust training matrix. The post holder will be required to use a computer, either stand alone or as part of a networked system, and will be responsible for the quality of information. The amount of time spent on this type of work will depend on the current needs of the caseload. Communications and Working Relationships To ensure CYP obtain information and optimal treatment, taking into account
social, cultural norms. To develop and maintain effective communication networks with service users, to support the enhancement of service delivery. To develop and maintain an effective and comprehensive communication network within integrated childrens services (health, social care, education, acute care, hospices and the voluntary sector) and other external agencies to ensure the best service for CYPs and their families. To promote and advocate for the equality, diversity and rights of children & young people who access the service. Demonstrate sensitivity towards CYP and their families by respecting the privacy of their home environment and maintaining professional boundaries at all times.
