Full Job Description

We are recruiting for a Multi-Disciplinary Team Coordinatorwho shares our ambition and vision to deliver theBest of Care by the Best Peopleand has the drive to take us forward, by ensuring that the Trust continues to be the first choice for patients and our staff., Develop a knowledge and understanding of the patient pathway for individual tumour sites. Use this knowledge to support and facilitate the MDT by ensuring all highly suspected, newly diagnosed, recurrent and subsequent cancer patients will be reviewed by an MDT meeting within the appropriate timescales.

Lead, co-ordinate, administrate and attend the MDT meeting. This includes producing and circulating lists of patients to be discussed, compiling an agenda and ensuring test results and other relevant information is taken to the meeting and ensuring that MDT members are aware of the breach dates at discussion.

Record outcomes and treatment plans as agreed at the MDT meeting in real time within Cancer services guidelines and documented on Infoflex. Ensure minutes for each patient areproduced, accurate and distributed to core members, filed in the hospital notes and distributed to named GPs within 24 hours.

Ensure that any follow-up actions from MDT meetings are carried out; including outpatient or diagnostic test appointments, tertiary referrals and treatment start dates according to all national 31 and 62 day targets.Ensure that all cancer target patients MDT outcomes/treatment plans are progressed in accordance with all National 31 and 62 day Cancer Waiting Times targets.

To lead and collaborate with other departments within and outside of the Trust to ensure effective co-ordination between all members of the MDT.
