Are you passionate about person centred care? We have an exciting opportunity for a support worker to join our Clinical Nurse Specialist Team as a Cancer Navigator. We have partnered with Macmillan to develop a cancer support navigator service. You will be supporting the Clinical Nurse Specialist in providing person centred care acting as a key point of contact for the patient, signposting to other services, providing information and facilitating holistic needs assessments., + Develop communication skills in order to convey complex and sensitive information effectively to patients, carers and other staff, overcoming any barriers to understanding and providing support during distressing or emotional events
+ Communicate with people in a form and manner that is consistent with their level of understanding and culture, and recognise barriers to effective communication
+ Communicate clearly, effectively and appropriately with the multidisciplinary team, patients and their family/visitors/carers
+ Respond appropriately to queries, take phone messages and pass on written and verbal information to patients
+ Provide relevant information to patients, family/visitors/carers and colleagues
+ Complete documentation accurately and contribute to reports of patients' activity and progress, recognising that the CNS has personal accountability for entries to patient records made by others under their supervision
+ Provide feedback on patient care and/or relevant patient communications to relevant healthcare professionals

We are seeking an experienced Health Care Support Worker with excellent communication, problem solving and organisational skills., The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Please be advised that there is a temporary top up for Bands 1,2 and 3 to reflect the incorporation of the top up to the living wage of £12 per hour - £23,465 per annum. This temporary top up will be in place until the annual pay uplift for 2024/25 is confirmed
