Life Planner (Link Worker)

We're looking for a dynamic qualified health or social care professional to be our Community Life Planning Coordinator, we will consider a candidate without a recognised qualification who can demonstrate the relevant experience. You will be part of a small, yet expanding and innovative team, making real, positive changes in peoples lives. You will empower clients to identify, shape and access meaningful activities to improve feelings of well-being (i.e. social prescribing).

You will work with our Head of Life Planning and our Administrator while gaining experience to take responsibility for existing Partners (clients) as well as new referrals. You will often work closely with the volunteer team to successfully match volunteers with Partners to enable support in the community. In addition, you will work in partnership with key stakeholders to compliment health and social care initiatives in response to the Care Act. The new Life Planner will need to maintain and further develop successful working relationships with Medway Council Adult Social Care teams, Medway Community Healthcare, Public Health and CCG to facilitate referral pathways and timely responses to emerging situations.
