Full Job Description

Responsible for providing expert knowledge of the NHS Continuing Healthcare Framework Support the Head of CHC in providing advanced theoretical and specialist knowledge of the national strategies surrounding the continuing healthcare framework Prioritise and manage workload effectively and change at very short notice in response to service issues Responsible for identifying and appropriate management or referral of any safeguarding and quality of care issues and take appropriate action to ensure that peoples welfare is protected Provide expert advice and guidance on application of Mental Capacity Act & DOLs, Power of Attorney and Court of Protection Work collaboratively with senior health and local authority colleagues, including social care and non-NHS organisations, providers of specialist health care services in the independent sector to ensure CHC placements are value for money and provide high quality care. Ensure complex and challenging problems are evaluated and negotiated solutions
implemented to ensure individuals are in the most appropriate placement Leads, influences and negotiates across statutory and non-statutory organisations as required, with support from the Head of Service Provides inspirational leadership to designated team of nurses and is responsible for line management and individual performance reviews Provide line management and visible leadership to staff members as delegated by CHC Head of Service Provide guidance, information and support to service users, carers and families Lead on strategic planning across the adult continuing healthcare pathway Disseminates sensitive and complex information to team members to ensure individual placements are appropriate and timely, ensuring that national and local CHC policy is adhered to Demonstrates good negotiation and influencing skills, with the ability to represent the team at national, regional and local level Leads on a variety of projects, managing deadlines to ensure the delivery of the CHC team
objectives Investigates complaints and takes appropriate action. Responsibility for the recruitment and retention process for the service Responsible for writing and implementing policies within own area of practice. Participates in relevant work streams within the local health and social care economy Provide expert training and education opportunities with the team, within the provider trusts, within local and national areas Work in collaboration with others to plan and deliver interventions to meet the learning and development needs of their own and other professions Advocate and contribute to the development of an organisational culture that supports continuous learning and development, evidence-based practice and succession planning Actively communicates research to colleagues and multi-disciplinary team Actively undertakes collaborative clinical audit and research with others e.g. development of referrals pathways within acute and community and develops protocols for client care based
on up-to-date research findings Demonstrates excellent interpersonal and communication skills, including presentation and report writing skills on a daily basis Provides and receives complex and sensitive information orally, in writing or electronically to inform work colleagues, patients, clients, public, external contacts Work collaboratively to maintain effective communication channels with patients and relatives and all disciplines of staff e.g. colleagues, consultants, key stakeholders and partners. Ensures that all documentation is completed accurately, is legible and falls within the NMC and Record Keeping Guidelines Leads in the development of written guidance and information to help meet patients need for information Participate in CHC appeals processes and provide responses to the outcome of continuing healthcare assessments, using a high level of communication, negotiation, and persuasion Demonstrates knowledge of theoretical frameworks underpinning nurse
specialist practice Displays knowledge of current structural changes in the NHS, and their impact upon service delivery Provide regular clinical supervision to staff within the continuing healthcare team
