Full Job Description

This is a new role to support the delivery of a robust surveillance pathway for Hepatocellular Cancer for the patients of Tameside & Glossop Integrated Care NHS Foundation Trust.
The Hepatology Pathway Navigator (HPN) will support the Liver Team in proactively tracking patients with liver cirrhosis, diagnostics pathways, and pre/post liver transplant to ensure they are appropriately managed and progress tracked +/- discussed at the weekly Liver MDT meeting.
The post holder will be responsible for creating and monitoring a database of liver patients to support their clinical pathway in ensuring they are booked in the appropriate time frame for tests and clinical review appointments.
Under the direction, guidance and supervision of the Hepatology CNS team and Consultants, the post holder will coordinate the care and monitor surveillance pathways for patients with cirrhosis enrolled in Hepatology HCC Surveillance at Tameside Hospital.
The post holder will also undertake pathway navigator duties and have a specific focus to provide direct support for patients to maintain engagement in surveillance pathways ensuring they receive surveillance scans on schedule., + The post holder will act as the patient's navigator tracking their timed pathway against the national guidance.
+ The post holder is responsible for supporting patients and their families through the relevant cancer pathway across secondary, community and primary care as required by the patient. They will be responsible for coordinating care, education, and support for people with complex care needs and providing a point of access to patients and carers.
+ The post holder must be adaptable and willing to work flexibly as the service progresses.
+ They will be responsible for entering accurate data onto the Trust's Electronic Patient Record and inputting into NHSE Liver Surveillance Database., General
+ Coordinate care by providing a single point of access, including rapid re-entry into the system for those people identified as having urgent needs
+ Provide advice and refer on or sign-post to other sources of support
+ Coordinate diagnostic and monitoring through appointments and investigations
+ Coordinate face to face appointments for those with complex results.
+ Discuss with patients issues relevant to their health and social care and provide support where possible or escalating relevant clinical teams if necessary.
+ Maintain accurate patient records on both the Trust Electronic Patient Record and the Somerset Cancer Register to reflect the care given.
+ Contribute towards the maintenance of local and Trust wide standard of care
+ Inform the clinical team/person in charge of:-
+ any changes in the patient's physical condition or situation
+ any significant factors relevant to the patient.
SECTION 2- KNOWLEDGE AND SKILLS
Communication
+ Ensure a smooth transition of care along the clinical pathway ensuring clear communication between the patient, clinical nurse specialist, consultants and both internal and external clinical support teams.
+ Maintain confidentiality of patient information
+ Contribute to the effective communication process between patients, carers, family, friends, and staff colleagues
+ Support people to access appropriate information and support by sign posting to a range of support services and take an approach which helps people to self-manage where appropriate
+ Maintain clear, timely communication with the relevant Cancer MDT Coordinator/ Tracker and to attend and participate in MDT meetings when required
+ Contribute ideas to the multi professional team
+ Support the delivery of patient and carer training and education
+ Encourage and support healthy lifestyle choices.
Key Relationships
+ Internal:
Lead Cancer Nurse, Cancer Clinical Nurse Specialists, Respiratory Consultants, Doctors, Cancer Management team, MDT Coordinator / Cancer Care Coordinators, Tracker team, Clinical Support teams, Directorate Management team and Macmillan Information and Support team.
+ External:
Patients, Patient families or carers, other Trusts that provide diagnostics or treatment for patients under surveillance and GP/Community services.
