Full Job Description
This role involved working in a variety of different settings to support people to make lifestyle changes to improve their health outcomes. This includes delivering the NHS Health Check service and supporting on lifestyle services.
The ideal candidate is well organised and has good people skills. They should be able to be mobile across the borough and have a willingness to learn and develop new skills and abilities.
