Full Job Description

This is an exciting opportunity for a Band 7 Occupational therapist or Physiotherapist to join the Central IHC falls team as the team lead on a permanent basis. The post holder should have relevant experience working in acute and community settings with a passion in the area of falls.The post is for 37.5 hours per week., The remit of the post is to develop and maintain a data base of patients referred to the service across the area, ensuring base line data and outcomes are available to evaluate the service for future development. The post holder will also be expected to deliver training around the prevention and management of falls prevention in care homes and other sites and to undertake falls groups, visits to patient's homes to complete assessment and intervention as appropriate. The post holder will attend relevant falls service meetings and supervise the falls practitioners and junior members of the team.
