eHub / Digital Transformation Manager

H&F Partnership is a single organisation PCN, and as such, we are able to build central support functions to enable the whole PCN. The eHub is a collection of clinicians from 5 sites Brook Green Medical Centre, North End Medical Centre, The Bush Doctors, Richford Gate Medical Practice and Park Medical Centre and an administrative team that process all e-consults (online consultation requests from patients) through a service called PATCHs Mon - Fri. These are either actioned by the administrative team, a pharmacist, a physiotherapist or a GP, or at times need to be booked into the site directly for a phone call or face to face appointment. The E-Hub/digital transformation manager will work across all 5 surgeries to build a single and consolidates process for digital requests from patients and also work with all the reception teams to improve triage., The E-Hub/digital transformation manager will work across all 5 surgeries to build a single and consolidates process for digital requests from patients and also work with all the reception teams to improve triage.
