Full Job Description

An exciting opportunity has arisen for an enthusiastic, motivated and committed Community Practitioner to join the current Regulated Care Team within Intensive Home Support on a fixed term basis covering covering maternity leave.

The role Community Practitioner is to be a clinical asset within the regulated care setting, providing direct care, education, and training to patients and care providers. Using advance care planning working with MDT & care providers this role will be responsible for the Holistic Care of patients within Regulated Care Settings.

The post holder will review hospital discharges back to regulated care settings and complete their holistic health assessment to ensure patient safety, this includes medication reviews and frailty. Some of these patients will already have assessments completed, utilising the MDT and own knowledge ensure that the appropriate care is being provided., Please see the attached job description and person specification for full details and main duties of the role.

  • Active Professional Registration & qualification i.e. RN, SW, AHP

  • Degree level qualification or equivalent knowledge or experience in relevant field., Evidence of clinical caseload management with complex cases in a range of clinical settings including community/primary care.

  • Proven experience of undertaking complex assessments

  • Continuing engagement in professional development and of reflective practice as evidenced in a professional portfolio

  • Evidence of influencing / negotiating in order to achieve change

  • Multi-disciplinary / multi-agency working

  • Participation in audit and evaluation


  • Desirable criteria

  • Evidence of experience in a mentoring/teaching role

  • Experience in leading the change processes at an operational level, Evidence of extensive professional clinical Knowledge, skill and experience in long term conditions/frail elderly

  • Good understanding of systems in health and social care that are wider than immediate role.

  • An awareness and understanding of the factors that contribute to good health and local public health priorities

  • Knowledge and understanding of the principles of governance and its implications to their service area., A constructive, inquiring, flexible approach to others

  • Self-confidence to challenge traditional practice and the persistence to address difficult, enduring issues

  • Flexible working, including the requirement to work as part of a 8am to 8pm, 7 day-a-week service on a rota basis, Ability to work across sites as designated through service, You must have appropriate UK professional registration.

  • Please note that the salary advertised is for full time hours. If this post is less than 37.5 hours per week, the salary will be pro-rata. For posts on Agenda for Change pay-scales, new entrants to the NHS will normally commence on the first pay point of the relevant band.

  • Disclosure and Barring Scheme (DBS)

    The healthcare sector is exempted from the Rehabilitation of Offenders Act 1974.

    In line with other NHS organisations in the North West Region, the Trust is now passing the charge for undertaking a DBS check on to candidates in the event they are successfully appointed into the post for which they have applied. Candidates can choose whether to pay this over 1-3 months as a deduction from the monthly salary. However, if you are applying for a post as a BANK worker, the payment must be made in full at the time of employment checks.     The cost of an Enhanced Disclosure will be £42.90 and for a Standard Disclosure £22.90 which includes a small handling fee.
