Care Manager

Active Care Groups Care in the Home division continues to go from strength to strength ensuring we provide a safe, reliable and outcome-based care to all clients by our exceptional support workers, clinicians, and management team., We have an amazing opportunity for an experienced Care Manager with NVQ 4 or above in Health and Social Care to join our friendly and supportive team. The successful candidate will need to be registered with Social Care Wales or register within 3 months of starting the role. This is an exciting time to join our Care in the Home team as we continue to grow the division.
