Full Job Description

West Kent Primary Care would like to recruit a Cancer Care Coordinator on behalf of Tonbridge Primary care Network (PCN). This role will support the GP practice to achievie the NHS England Early Cancer Diagnosis Direct Enhanced Service by supporting multidisciplinary and practice teams in the coordination of pathways for patients with cancer. The Cancer Care Coordinator will work with patients ensuring they have the right support to understand their diagnosis and what the next steps are to help them during this distressing time. A major part of the role will be safety netting and checking that patients referred into the two week wait pathway have received an appointment with secondary care in an appropriate timeframe. The post holder will act as a point of contact for patients during their cancer journey signposting to services such as Macmillan Services.

West Kent Primary Care will act as employer for this role. We are a collaborative and ambitious GP Federation working with GP practices across Sevenoaks, Weald, Tonbridge, Tunbridge Wells, Malling, and Maidstone to improve General Practice provision in the area.
