Full Job Description

If you have a desire to support individuals to improve their health, then this is the ideal opportunity to gain valuable insight and knowledge. As an Apprentice, you will work towards building the knowledge, experience, and skills required of a Wellbeing Practitioner to be able to:

  • Work as part of a team rota covering all aspects of service, meeting clients in a variety of settings to identify their support needs through initial assessments. Updating all client interaction on the service caseload management system.

  • Provide ongoing support through personalised support planning specifically around smoking cessation, weight management, harmful drinking and may include advice and developing/delivering a programme of support. When needed facilitating access to ongoing support to broader holistic issues impacting an individuals health - e.g housing, debt etc.


  • In addition, you will participate in Wellbeing Service Promotional events in a range of venues and support colleagues who are delivering training in subjects including Smoking Cessation, Alcohol and Weight Management brief advice etc. You will also support the service Triage team by processing referrals and assessing new client's needs.

    With a desire to support individuals to improve their health and a willingness to learn about Public Health and of the wider determinants of health, you will have a knowledge and understanding of Portsmouth demographics. You will have good communication and interpersonal skills, data management and organisational skills. As well as good IT skills we are looking for someone who is flexible, adaptable, keen to learn and willing to undertake the Apprenticeship qualification as well as any other job specific training.

    You will have good numerical and literacy skills be a team player and the ability to work across the Portsmouth area., Are you a citizen of the United Kingdom or another country within the European Union (EU) or European Economic Area (EEA).

    Have you been ordinarily resident in the United Kingdom or another country within the EU or EEA for at least 3 years from current date. Do you depend on a visa or other permission to reside in the United Kingdom?

    People living in the most deprived communities in Portsmouth have poorer health outcomes and shorter life expectancy. Around two thirds of reduced life expectancy and health inequality is caused by four conditions (cardio-vascular disease, cancer, liver disease and respiratory disease). These conditions are primarily associated with the four main risk factors known to impact health - specifically smoking, excess alcohol, unhealthy weight, and physical inactivity. The Wellbeing Service supports individuals to modify behaviours, improve their health, increase life expectancy, and to reduce health inequalities for people in Portsmouth.
