Full Job Description

The successful candidate will be provided with a robust induction and personal development plan to meet their professional development needs whilst working closely with the Consultant Nurse and Consultant Body within the Frailty service.

The successful candidate will be based at Southport Hospital with the Frailty team. You will be given an educational and clinical Consultant supervisor who with the Consultant Nurse will provide regular guidance and feedback whilst developing your portfolio, working towards credentialing, and enhancing your clinical skills providing safe and appropriate patient care. This role will be pivotal in providing clinical expertise in the delivery of the future models of care for the service.

The post holder will deliver an 80% clinical component to their role and 20% related to the other pillars of advanced practice, education, research, and leadership/management. You will be provided with mentorship from both a Consultant in Advanced Practice and, you will be supported to become credentialed on the Frailty ACP curriculum.

The working pattern of the team is designed to meet the needs of the population accessing the service and the successful candidate will be expected to work some weekends and bank holidays on a rota basis as the service embeds. The service is currently operating 8am-4pm Monday-Friday.

We are seeking a highly skilled, enthusiastic, hardworking, and motivated clinician, who has excellent communication, organisational and time management skills to utilise within this busy team. The successful candidate will have highly specialist assessment and treatment skills and be able to assess, request and interpret appropriate diagnostics and to manage and signpost patients who access the Frailty service.

Qualified practitioners will be an advanced level, independent practitioner working autonomously using expert advanced clinical skills to diagnose, treat patients.

The Secretary of State has approved the partnership between St Helens & Knowsley and Southport & Ormskirk Hospitals.

From 1st July we have come together as a single organisation under the name Mersey & West Lancashire Teaching Hospitals NHS Trust.
