Full Job Description

Job Summary To actively contribute and participate in the provision of pharmaceutical care to the services as part of the medicines management team. To be responsible for developing, organising and managing the clinical pharmacy service to specific wards/unit/teams in conjunction with the Chief Pharmacist/ Pharmacist team leader oCurrently acute adult inpatient ward/rehab unit and crisis service. To participate as an active member of the specific ward/unit/clinical teams and ensure medicines management is optimal in the allocated areas. To provide clinical pharmacy leadership for an allocated sub-group of the Trust Drug and Therapeutics/medicines management committee (D&T/MMC)including the development of relevant clinical guidelines.

oThese include learning disabilities, clinical audit, education and training, lithium and clozapine. To provide a dispensary commitment from the hub pharmacy at Fieldhead, Wakefield and support the supply of pharmaceuticals within existing contracts. o These are currently with Dewsbury District Hospital and Calderdale Royal Hospital Duties and responsibilities Pharmacy Manage the development, implementation and monitoring of medicines management systems promoting economic, safe and rationale use of medicines across allocated wards/units/clinical teams. Support the supply of pharmaceuticals in line with all relevant legislation.

Participate in the dispensing and supply of medication to patients and work with the pharmacy technicians and assistants to ensure medicines are provided in a format suitable for the ward or in the community. Supervise the work of supporting staff. Work in line with the Trust Medicines Code Controlled Drugs Manual and pharmacy standard operating procedures (SOPs) ensuring full documentation of the processes. Make recommendations for improvement at the appropriate forum as relevant.

Act as the responsible Pharmacist while carrying out a regular dispensary slot at Fieldhead Pharmacy. Work as part of the pharmacy team to support a comprehensive clinical pharmacy service, relevant medicines information and a safe and effective supply of pharmaceuticals to the Trust. To ensure medicines reconciliation takes place in line with the Trusts medicines reconciliation policy, including full documentation of the process. Use RiO to document all relevant medicines management activities for individual service users.

Wards/Units/Clinical Teams To work as part of the multidisciplinary team to provide safe, clinically effective and cost efficient use of drugs. To support prescribing practice in line with National and Trust guidance. To give appropriate advice and support where prescribing is outwith normal guidance or unlicensed. To maintain a caseload of queries via the clinical queries mechanism and follow these up at regular intervals.

To regularly review, monitor and audit prescriptions and participate in potentially challenging discussion with Prescribers/Trust staff where the planned care is not conducive to good medicines management and to provide advice on alternative treatment options. Also to support other Trust staff in this role. To develop medicines management systems from admission to discharge and provide support in the community on the use of medicines. To provide information, education and training on the use of medicines on your allocated wards/units/clinical teams on all aspects of medicine management.

In particular to provide: oRapid tranquilisation training on the in-patient wards. oUpdates on clinical guidelines and the medicines code. oUpdates on the use of clozapine and other psychotropic medicines. oSafe medicines practice guidance.

Manage the use of Risperdal Consta in line with the Trust prescribing policy ensuring information is logged. Manage the use of clozapine in line with the SPC and Trust guidance supporting service users and healthcare staff to optimise the appropriate use of this complex drug in particular the Wakefield locality. To monitor the consent to treatment regulations and act as a second opinion consultee for the use of medicines where appropriate. Work as a team with allocated pharmacy technician and nursing staff to ensure: oPatient information on the use of medicines is available to service users and carers.

oSafe and secure storage of medicines appropriate to needs of the service users. oAccess to leave and discharge medicines. oA relevant stock list is maintained and costed and stock is checked every three months. Carry out induction for all new medical staffing to the allocated wards/units clinical teams and support the Trust induction programme.

Ensure pharmacist is embedded as part of the ward clinical team and linked to the productive ward process. Resource Management Assist the Chief Pharmacist in monitoring the medicines budget and advise and report on expenditure and medicines usage analysis to allocated clinical areas. To identify changes in prescribing practices that may impact on the prescribing budget. Ensure decisions made by the D&T Committee and the pharmacy team are carried out by prescribers and all healthcare professionals.

Make recommendations for improvement as relevant. Communication Report all incidents via Datix and contribute to the development of safe medicines management systems. Provide training to pre-registration pharmacists and diploma students Treat all service users, carers, pharmacy colleagues and all healthcare and Trust workers with dignity and respect at all times. Identify and respond to the complex pharmaceutical information needs of service users, carers and healthcare professionals.

Information will be required to be presented on an individual case basis and in groups. Trustwide Participate in the development of the medicines management clinical audit programme in particular the POMH audits and ensure roll out to allocated units. Provide the clinical pharmacy input to the identified sub group of the D&T. Participate in research and development as required for the development of the clinical pharmacy services.

Provide clinical supervision to pharmacy colleagues. To cover for other senior pharmacists in their absence. If required due to sickness or other absence to work in other localities of the organisation. To participate in Saturday morning, Bank Holiday and out of hours services.

General To observe the principles of Clinical Governance in general and in particular be able to demonstrate participation in Continuing Professional Development. This will include attendance at appropriate training courses. To carry out CPD in line with the RPSGB requirements. To maintain and demonstrate a good clinical pharmacist knowledge.

To work towards membership of the College of Mental Health Pharmacists. To be able to plan and organise their own workload and that of others within allocated working hours. To work within the policies and procedures of the South West Yorkshire Partnership NHS Foundation Trust and also to ensure compliance with the pharmacy policies and procedures from Barnsley, The Mid Yorkshire Trust and the CHFT when working in partnership. To participate in the Trusts Appraisal system.

Service improvement models within the Trust may result in the necessity to work in other areas of the Trust.
