Full Job Description

Clinical Responsibilities: Clinically examine and assess patient needs from both a physiological and psychological perspective, and plan clinical care accordingly through telephone triage or face-to-face consultations. Assess, diagnose, plan, implement, and evaluate interventions/treatments for patients with complex needs. Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long-term condition (as appropriate). Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a comprehensive management plan. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, within your scope of practice. Work with patients to support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed or over-the-counter medication, including medication
regimens, side-effects, and interactions. Prioritise health problems and intervene appropriately to assist patients in complex, urgent, or emergency situations, including initiating effective emergency care. Support patients in adopting health promotion strategies that encourage healthy lifestyles and apply principles of self-care. Utilise and demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment. Quality and Safety: Deliver a high-quality service that meets the needs of patients. Recognise and work within your competence and professional code of conduct. Produce accurate, contemporaneous, and complete records of patient consultations, consistent with legislation, policies, and procedures. Participate in and adhere to audit processes and quality improvement initiatives. Prioritise, organise, and manage your own workload to maintain and promote quality care. Assess the effectiveness of care delivery through self and peer review,
benchmarking, and formal evaluation. Initiate and participate in the maintenance of quality governance systems and processes across the practice and its activities. Use structured frameworks, such as root-cause analysis, to manage, review, and identify learning from patient complaints, clinical incidents, and near-miss events. Professional Development: Engage in continuous professional development and contribute to the training of junior staff and students. Participate in practice teaching sessions and multidisciplinary team (MDT) meetings to enhance team learning and patient outcomes. Take advantage of opportunities for external qualifications and continuous education to advance your skills and expertise. Team Collaboration: Work collaboratively with our multidisciplinary team, including 5 GPs, 3 other ACPs, 4 nurses, 2 physiotherapists, a clinical pharmacist, and a social prescriber. Participate in frailty MDTs and contribute to the overall team dynamic, ensuring effective
