Full Job Description

Monday to Thursday 9 - 5 pm, Friday 9 - 4 pm.

Driver required to take clients to activities.

Main purpose of position

· To share with other staff in meeting the needs of service users in a way that respects the dignity of the individual and promotes independence.

· Undertaking delegated responsibilities at the appropriate level.

Responsible to:

· Senior facilitators, team leader, manager, and partners.

Liaising with:

· Service users, Essex Care Consortium staff, managers, relatives and friends of service users, outside agencies, professionals, and members of the public.

Recruitment, employment, and training

· To complete Essex Care Consortium induction training for new staff within the first three months of employment.

· To attend, complete and keep up to date any personal training provided by Essex Care Consortium.

· To ensure all staff follow Essex Care Consortium rotas, reporting any short falls in staffing to the team leader/manager/on call manager.

· To provide residential emergency cover during your working hours.

· To ensure that you follow the Essex Care Consortium annual leave and sickness policies.

· To be aware of Essex Care Consortium Policy regarding staff conduct and dress code and ensure that all staff follow this.

· To be aware of Essex Care Consortium grievance and disciplinary procedures.

· To be aware of Essex Care Consortium complaints procedures and assist in ensuring these are followed appropriately.

Care of service users

· To assist with referral appointments and meetings regarding admission of service users, completing any necessary paperwork as required.

· To assist and support service users with their admission and discharge transitions.

· To welcome new service users and their relatives to the Learning and Development Centre

· To assist the service users with all aspects of personal care, ensuring that the core values of care are always promoted:

Privacy, dignity, respect, fulfilment, choice, independence, self-esteem, safety

· To ensure the following are maintained to Essex Care Consortium standards:

activity rooms

monitoring of health

personal hygiene.

· Where necessary, to take responsibility for designing session plans following the set criteria in such a manner that individual progress and achievement may be evaluated and
correctly recorded.

· To take responsibility that any activity is correctly organized, and staff are given the necessary information required to ensure the needs of individual service users/groups are being taken into consideration and catered for.

· To liaise with staff and management to ensure that session plans are informative, instructive, and appropriate.

· To be always responsible for ensuring that service users' needs are being met by the service as laid down in Goal Plans and Care Plans where appropriate.

· To encourage and motivate service users to build upon existing skills and, wherever possible and appropriate, develop new ones.

· To provide guidance, instruction, and the necessary level of supervision in order that the service user can understand and complete an objective in such a manner that a sense of achievement is reached.

· To be aware that the process of learning is two-fold on the part of the student and the
staff member.

· To assist with the appropriate administration of medication following procedures set out in the staff handbook and endorsed by Boots Monitored System and training.

· To act as link worker, keeping the Link worker Folder up to date and carrying out all responsibilities described in the Link worker Role.

· To produce goal plans in conjunction with service users following guidelines provided by Essex Care Consortium ensuring that each Goal Plan contains an up-to-date copy of the following:

Looking after myself plan, infringement of rights and risk assessments.

Getting out and about plan, infringement of rights and risk assessments

Understanding myself plan, infringement of rights and risk assessments.

Care Planning Contract

Care Plan Evaluation.

· To produce a review report in conjunction with the service user and all those involved, following Essex Care Consortium guidelines.

· To ensure service users wishes and opinions are listened to.

· To ensure a wide range of activities are offered to service users with appropriate staffing.

· To support service users within their home following all relevant guidelines.

Protection of service users

· To be aware of and ensure the appropriate use of the following procedures and policies:

all policies and procedures contained in the protection of service users' section within the Essex Care Consortium Staff Handbook

Essex County Council Vulnerable Adults Policy

Essex County Council Guidelines for the Protection of Children.

· To report to the senior, team leader or manager anything, however minor, that is a cause for concern regarding the care of a service user.

· At all times, listen and take notice of what a service user is saying or communicating.

Emergency procedures

· To be aware of and understand all the emergency procedures contained in the handbook (e.g. buzzer system, fire alarm system, generators etc.)

· To be aware of and in the event of an emergency or crisis follow guidelines and procedures set out by Essex Care Consortium.

· To carry an emergency buzzer, ensuring that other members of staff also carry one.

· To ensure you follow Essex Care Consortium policy on physical intervention management and restraint contained in the staff handbook.

Communication

· To ensure you work within a total communication framework.

· To ensure all staff liaise with each other, service users, managers, relatives and outside agencies in an appropriate and professional way.

· To ensure all required documentation and minutes of meetings you have taken are completed appropriately and kept up to date.

· To ensure you always maintain appropriate confidentiality with regard to all aspects of their work.

· To attend appointments and meetings as required, ensuring all appropriate preparations have been made e.g. reports, review packs, history etc.

· To attend service user or staff meetings as requested by team leader or manager in line with Essex Care Consortium policy.

· To answer telephones and welcome visitors in an appropriate and professional manner, being aware that you are not only a representative of Essex Care Consortium but more importantly an advocate or spokesperson in certain circumstances for the service users.

Finance and resources.

· To be given support with identifying and assessing the resources needed and available to carry out planned activities and visits off the premises e.g. staff levels, skills, special equipment, mobile phone, support network.

· To ensure that the resources available within the Learning and Development Resource are managed appropriately e.g. lighting, heating, water, and equipment.

· To ensure any service users who are working with budgets are fully supported.

· To liaise with the appointee regarding service user's financial affairs.

· To liaise with Essex Care Consortium Bookkeeper regarding matters concerning salaries, National Insurance etc.

· To be accountable for monies handled on behalf of service users or Essex Care Consortium e.g. retaining receipts.

Health and safety

· To ensure you adhere to and understand regulations regarding Health and Safety, hygiene and COSHH as written in the staff handbook.

· To ensure you are aware of the due diligence folder.

· To ensure you use equipment provided appropriately and always adhere to health and safety regulations e.g. self-assessment of risk taking, not propping doors open, ensuring appropriate signs are in place on wet floors.

· To report any hazards or concerns to your senior, team leader or manager and ensure problems are rectified.

· To take charge in the event of an emergency, minimizing and assessing all risks and informing appropriate services, managers etc.

· To be aware of and report any repairs or damage to the senior, team leader, manager, health, and safety officer using the appropriate method e.g. maintenance reports, verbally.

· To ensure you always follow the Essex Care Consortium drivers and vehicles policy.

· To ensure service users are not left unsupervised by facilitators or support workers whilst internal maintenance people or external contractors are working within the home.

Regulatory checks

· To be aware of and ensure all the appropriate policies, procedures and legislation are followed regarding checks. Records are maintained, updated, and completed e.g. water temps, fire alarms system, emergency lights, fridge temps.

· To ensure you understand and complete the necessary fire drills with service users as set down in the Essex Care Consortium Staff Handbook.

· To ensure all equipment is PAT tested annually with updated stickers.

Quality assurance

· To be aware of the Quality Assurance systems in place at Essex Care Consortium and assist the team leader/manager in maintaining the standards e.g. Investors in People 2002.

· Fully cooperate with the Quality Assurance Officer regarding their auditing procedure.

Other procedures

· To carry out other reasonable duties as identified by yourself, the senior, the team leader/manager to maintain standards of care and the smooth-running Essex Care Consortium.
