Full Job Description

We have an exciting opportunity for a Ward Discharge Co-ordinator to join our friendly and dynamic team at Bishops Castle Community Hospital.
The role involves working within the ward environment as part of the multidisciplinary team, to provide a continuous coordinated approach to the safe and timely discharge of patients with simple and complex discharge needs.
Please note this vacancy may close early if a high volume of applications is received, we advise you submit your application early to avoid disappointment., + Communicate and liaise with patients, their families and carers and other key external stakeholders who play an integral role in patient discharge.
+ Support the delivery of the SAFER red to green approach to expediting patient care.
+ The role requires a high proportion of autonomous working with in the ward team within the sphere of the role.
+ Attend ward round, organise and chair MDT meetings and take action to expedite investigations and treatments to support pathway progression using the SAFER principals of red to green.
+ Liaise with other members of the wider interdisciplinary team i.e. social services, ICS, district nurses, specialists nurses and other relevant external agencies to facilitate timely and co-ordinated patient discharge.
