Full Job Description

  • Engage with people who have severe and enduring mental health issues to develop plans to move towards independence.

  • Offer tailored and holistic 1:1 support, enabling people to develop their independent living skills.

  • Embrace the 'whole team approach', working with colleagues and partners to reduce risks, support people to meet their goals and build aspirations.

  • Be responsible for good quality housing management, including rent and arrears management, health and safety quality assurance/compliance.

  • Supporting people to move on to appropriate accommodation, and to access education training and employment (ETE).

    Excellent communication skills

  • An empathetic and trauma-informed approach

  • The ability to work in collaboration, with the people in the services, your team, internal and external organisations

  • Skills to ensure accurate data collection, recording and maintenance of an up-to date case management system

  • Working effectively with people that have a range of support needs using a person centred, outcome-based approach

    Thames Reach runs projects and services that help to end homelessness in London. We work directly with homeless people on the streets and in hostels. We work to prevent vulnerable people from becoming homeless. We help formerly homeless people get their lives back on track. We help people find work and access training and volunteering opportunities.
