Full Job Description

An exciting opportunity has arisen for a motivated and enthusiastic band 6 Occupational Therapist to be part of the AMH Community Rehabilitation Team for Solent NHS trust.

You will be part of the Community Intensive Rehabilitation Team (CIRT) which provides a range of rehabilitation focused assessment and intervention for adults with severe and enduring mental health problems who are based in the community. You will work alongside a welcoming and friendly multi-disciplinary team consisting of Occupational Therapists, Social Workers, a psychology lead, and experienced support workers. They have responsibility for formulating intervention plans and delivering specialist individual and group work interventions for a caseload of clients with complex needs.

The Community Intensive Rehab Team (CIRT) offers intensive rehabilitation for adults with complex psychosis and related severe mental health conditions. CIRT aims to ensure that people receive rehabilitation when they need it and promotes a positive long-term approach to recovery.

The CIRT can work in all community settings; though will commonly work with people living in supported accommodation, often long term, in order to enable their optimum level of functioning and independence. CIRT will support people to develop structure and purpose in their day to day lives and promote a sense of community and belonging.,

  • Act as a key resource for the Occupational Therapy needs of the wider MDT caseload.

  • To guide the team on the appropriate use of therapeutic risk-taking during MDT discussions, using specialist OT and mental health knowledge.

  • To meet the needs of service users in an effective, efficient and recovery focused approach.

  • To ensure the maintenance of a high standard of care to working age adults with severe and enduring mental health difficulties supporting the implementation of the Care Programme Approach (CPA) and act as Lead Professional.

  • Provide occupational focused assessment, formulation, care plan, and intervention to patients with complex functional needs in their own homes and communities.

  • Evaluate Occupational Therapy interventions and consider ongoing needs of the patient and identity opportunities for longer term support when required.

  • Chair care programme approach meetings, case conferences and other clinical meetings as indicated.

  • To develop, facilitate and evaluate a group work programme.

  • To support the wider Occupational Therapy service when required; this may require you to work with patients in different services.

    We have been working with other local NHS Trusts to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight. Subject to approvals, the aim is for the new Trust to be formed by July 2024.Project Fusion is the name for the programme to create the new combined NHS Foundation Trust. Hampshire and Isle of Wight Healthcare NHS Foundation Trust will be bringing the services into a single organisation will result in more consistent care, more equitable access to services irrespective of postcode, and a more sustainable workforce and services. The new organisation will operate locally to ensure services can best meet the needs of different communities.The new Trust will be comprised of all the services currently provided by Solent NHS Trust and Southern Health NHS Foundation Trust, the community, mental health and learning disability services provided by Isle of Wight NHS Trust and

  • child and adolescent mental health services delivered in parts of Hampshire by Sussex Partnership NHS Foundation Trust.Colleagues will transfer to the new organisation under Transfer of Undertakings (Protection of Employment) -- known as TUPE on 1st July 2024. TUPE is a legal process to ensure you transfer out of one organisation and into another, maintaining the terms and conditions of your employment.

    £35,392 to £42,618 a year Please note for part time hours the salary will be pro rata.
