Full Job Description

The post holder will be based on Reablement Unit and will be responsible for co-ordinating daily discharges and achieve early movement into vacated beds from acute wards to support patient flow.The post holder will attend all MDT meetings to establish an accurate discharge profile for the day, and will work with MDT to ensure that all patients are safely discharged to the appropriate place of care in a timely manner, utilising the discharge lounge where appropriate.

The post holder will be expected to utilise operational knowledge and administrative skills as necessary to undertake this role in order to assist clinical and non-clinical staff member They will be expected to liaise regularly with operational site team by providing regular updates on confirmed discharges; further potential discharges; and bed availability.

    Sutton Health and Care (SHC) is an innovative joint venture launched in April 2018 between local partner health and social care organisations, in which Sutton Community Services will be fully integrated to support the deliver a new care model for the people of Sutton.
