We are looking for a Band 5 Community Nurse to work within our St Neots Team.
In these roles, you would be working at part of the Neighbourhood team which includes District/Community Nurses, Physiotherapists, Occupational Therapists, Mental Health Nurses and Team coordinators. Linking closely with colleagues including general practitioners and other members of the multidisciplinary team to support excellent nursing care at home, reducing the necessity for acute hospital admission., + Keeping the patient at the centre of care, ensuring patient and carer participation in decision making
+ Providing a high standard of care
+ Providing a high standard of nursing intervention within a patient's own home, with access to Specialist and Advanced Practitioners when required
+ Working as part of a 24/7 service and multidisciplinary team

    + To be accountable for assessing interpreting, planning, implementing and evaluating treatment to patients within professional guidelines
    + To keep the patient at the centre of care, ensuring patient and carer participation in decision making
    + Provide support and education of peers, new staff, non registered staff, and students
    + To provide high standards of nursing intervention within a patients own home, this includes lone working , with access to specialist and advanced practitioners when required

    Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence
