Full Job Description

The post holder will be responsible to the Service Manager of Older Adult Community Mental Health, Dementia & Frailty (Mid & South Essex). The post holder will be responsible for the management of the FIRST (Functional Intensive Support and Response Team) admission avoidance, crisis and home treatment pathways, providing Leadership, supporting staff and working collaboratively with Consultants Psychiatrist in Mid & South Essex, other grades of Doctors, Allied Health Professionals and Social work Practitioners to provide and deliver a high quality and safe care.
FIRST is a service working from three localities South West Essex (Basildon, Brentwood and Thurrock), Mid Essex and South East Essex (Castle Point, Rochford and Southend. Each locality has a clinical team manager, with the wider service is supported by the MSE Pathway lead, all working together for strong clinical management and decision making across MSE ICS.
FIRST will align with DIST (Dementia Intensive Support Team) in South East Essex and the post holder will provide management support to DIST as the alignment progresses.

Main duties of the job
The post holder will demonstrate critical thinking in the clinical decision making process, including joint working with the Multi-Disciplinary Team to meet the needs of the patients, and carers to ensure that they are working to EPUT Clinical Policies and Procedures, Nursing and Midwifery Council Code of Practice and Standards. In delivering your duties you are expected to display behaviours that are aligned with the Trust values and as requested in accordance with the NHS Constitution at all times.
You will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services; providing a 7-day a week, 24-hour a day service for 365 days a year., To work collaboratively with Inpatient and Community (Mental and Physical Health) services in order to aid the Patient's recovery and Well-being plans in the community.
To ensure safe Clinical Practice takes place and maintained to Care Quality Commission standards at all times and in line with the NHS Constitution.
To work in conjunction with the Consultant Psychiatrists, to ensure that patients are supported in the most appropriate clinical practice environment from a Multi-disciplinary perspective
To create appropriate pathways within sphere of responsibility and work closely with the Assessment and Treatment units to provide a seamless service when an admission is warranted.
To manage and deliver effective and safe services which support the key elements of Clinical/Practice governance. The post holder will focus on leading the teams to introduce ways of continuously improving the quality of their services and safeguarding high standards of care by creating an environment in which excellence will flourish.
