Full Job Description

Are you a prescriber with minor illness experience, looking for a new challenge?   • Provide assessment, diagnosis & treatment at first point of contact by attending to patients according to their needs in the practice. Act as an autonomous clinician, with the support and guidance from the GPs

  • Proactively identify, diagnose & manage treatment plans for patients at risk of developing a long-term condition (as appropriate).

  • Diagnose & manage both acute and chronic conditions, integrating both drug & non-drug based treatment methods into a management plan.

  • Accurately triage & prioritise patients, including in emergency situations, demonstrating the use of a variety of techniques to elicit the history of an event/illness, including past medical and drug history.

  • Recognise, assess & refer patients presenting with mental health needs

  • Telephone triage as appropriate for clinics & home visits & provide access to GPs as necessary.

  • Evaluate clinical information from examination & history taking & initiate appropriate treatment &/or referral, including admission to hospital.

    All applicants should have the following essential qualifications, registrations, skills and experience:
  • Experience of dealing with vulnerable patients

  • Be registered on one of the below:

  • Registered adult nurse, registered with the Nursing & Midwifery Council (NMC)
    Pharmacist registered with the General Pharmaceutical Council (GPhC)
    Registered Paramedic with HCPC
    RCN Accredited Nurse Practitioner Module
    Registration as an independent non-medical prescriber

    The following qualifications are desirable:
  • BSc (Hons)

  • Mentorship qualification
