Are you a prescriber with minor illness experience, looking for a new challenge? - Provide assessment, diagnosis & treatment at first point of contact by attending to patients according to their needs in the practice. Act as an autonomous clinician, with the support and guidance from the GPs
- Proactively identify, diagnose & manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
- Diagnose & manage both acute and chronic conditions, integrating both drug & non-drug based treatment methods into a management plan.
- Accurately triage & prioritise patients, including in emergency situations, demonstrating the use of a variety of techniques to elicit the history of an event/illness, including past medical and drug history.
- Recognise, assess & refer patients presenting with mental health needs
- Telephone triage as appropriate for clinics & home visits & provide access to GPs as necessary.
- Evaluate clinical information from examination & history taking & initiate appropriate treatment &/or referral, including admission to hospital.
- Experience of dealing with vulnerable patients
- Be registered on one of the below:
- BSc (Hons)
- Mentorship qualification

All applicants should have the following essential qualifications, registrations, skills and experience:
Registered adult nurse, registered with the Nursing & Midwifery Council (NMC)
Pharmacist registered with the General Pharmaceutical Council (GPhC)
Registered Paramedic with HCPC
RCN Accredited Nurse Practitioner Module
Registration as an independent non-medical prescriber
