Full Job Description

  • New client assessments

  • Maintaining care plans; including care management on OneTouch

  • Resolving alerts/Reducing alerts on the OneTouch system

  • Care plan reviews and improving the quality of existing care plans

  • Actioning audits carried out by the manager each month

  • Complete weekly reports for the Registered Manager to be made aware of and sign off

  • Support Homecare staff regarding their duties outlined in the care plan

  • Liaise with our Care Coordinator to ensure staff/client compatibility

    Has experience of undertaking care assessments

  • Ideally holds a minimum of an NVQ level 3 or is suitably qualified by experience.

  • Is compassionate and caring.

  • Will always represent Alcedo Care in a professional and dedicated manner

  • Can communicate well

  • Is computer literate

  • Is hardworking and flexible

  • Holds a full UK driving licence and has access to their own car., Disability Confident

    Alcedo Care Warrington, part of the Alcedo Care Group, which is one of the fastest expanding health care providers in the North West Region is looking for an experienced Home Care Assessor to join its professional and dynamic Management Team.

    We are looking for someone who is experienced in carrying out assessments of potential new clients in their own homes or other care settings.

    You will be responsible for meeting with the clients, their relatives and possibly other healthcare professionals. You will fully assess their requirements and the support they require and develop a person-centered, comprehensive care plan.

    We use the OneTouch electronic monitoring system which is fully integrated with all aspects of our client's care plan and it will be your responsibility to ensure this is kept up to date, to an exceptionaly high standard and ensure the system is at 100% compliance at all times.
