Full Job Description

We have an exciting opportunity for a rehabilitation focused physical health Therapy Assistant Practitioner based in East Cambridgeshire working between the North Cambs Hospital, Wisbech and Princess of Wales Hospital, Ely.

The successful applicant will work between two adult inpatient rehabilitation wards alongside therapy and MDT colleagues and will have the opportunity to follow-up patients in the community following discharge or to support the ICT community therapy pathway.

Applicants will be delivering prescribed therapeutic interventions and treatment, providing equipment, and supporting discharge planning alongside the wider MDT and qualified therapy staff. This position offers flexibility, variety, and the opportunity to be involved in the transformation of our intermediate care services.

This is a new operating model of flexibility, merging our community and in-patient therapy staffing to improve patient outcomes with a seamless therapy pathway. If you are flexible, caring, and keen to learn then please apply.,

  • To work primarily across Pathway 2 (In-patient Rehabilitation units) and to support Pathway 1 (care at home) for the Intermediate Care service.

  • To work autonomously under the direction of a registered practitioner within the Intermediate Care Team.

  • To keep the patient at the centre of care, ensuring patient and carer participation in decision making.

  • To always provide a high standard of care

  • To have delegated responsibility for assessing, interpreting, planning, and implementing therapy interventions for specific groups of patients.

  • To use clinical judgement to evaluate patient progress and update goals as required.

  • To use outcome measures as directed by service

  • To provide advice to patients and carers

  • To supervise and support education of Integrated Care Workers (Band 2 and 3) deliver high quality patient care.

  • To plan and prioritise own workload with input from wider therapy team and service leads

  • To provide resilience and cover to other locality teams as required due to operational demands and system pressures
