Advanced Nurse Practitioner

Perform diagnostic and prognostic investigations following clinical pathways, for example, DVT, PE, etc. Manage unpredictable patterns of work, accepting referrals from health care professionals Work closely with Medical and Nursing professionals Allied to Medicine teams in the trust to facilitate the prompt evidence based treatment of patients Triage patients prioritizing clinical need and highlighting patients who require immediate medical intervention to the medical team Act as a resource for all members of the multi-disciplinary team, providing advice regarding aspects of managing ambulatory sensitive conditions Work with medical and nursing colleagues to provide a good standard of holistic care for patients. To coordinate within the unit, taking referals and lead the staff on duty ensuring patient safety. Work with Ward Coordinators and Patient Flow to provide emergency admissions access to the service Assist the team to develop innovative and flexible approaches to care.

Ensure accurate and legible nursing records are kept in accordance with trust policy and NMC Assisting in developing clinical protocols/guidelines and standards to facilitate practice developments. To assist the Advanced Nurse Practioner to facilitate the development of the clinical skills of the nursing and medical team, contributing towards the ongoing education and development needs of the unit. Support the development of an integrated approach to the delivery of Acute Medicine and Emergency Care. Deliver & promote best evidence based practice
